Dental Assistant Checklist

1. Employment History

  • Dental Assistant application form
  • Current CV
    • include all employment history since you have gained your initial qualification (please ensure there are no gaps)
    • must be able to demonstrate a minimum of 250 clinical hours annually
    • if you do not work full time, you will need to provide a Statement of Service (SOS).

2. Personal Clinical Skills and Qualifications

  • A copy of your Certificate III in Dental Assisting (or equivalent qualification)
  • Evidence of CPR completed in the past 12 months – inc. practical and theoretical component
  • Accrued six months of full-time experience in a relevant clinical setting after graduation
  • Verification of Qualifications Form
  • Evidence of competency in Sterilisation completed in the last 12 months
  • Evidence of competency in Infection Control completed in the last 12 months

3. Proof of Identity

  • Provide two forms of identification including:
    • a certified copy of your current, unrestricted driver’s licence
    • a certified copy of your current Australian/New Zealand passport or full birth certificate (please provide both if you don’t have a driver's licence).

4. Photo for RAHC ID

  • A colour passport style photo for your RAHC ID card (must be emailed or posted)

5. References

  • Provide two completed Reference Check Forms from:
    • a manager you who you have worked with clinically for at least 3 months, and
    • a manager or peer who you have worked with clinically for at least 3 months.

6. Working with Children clearance application

  • Please visit the website below at SAFE NT and follow the procedure to complete your Working with Children Clearance Application.

    A Working with Children Clearance is mandatory to work in the Northern Territory and must be completed (or an exemption obtained) before a placement can take place. This process should take between 4-6 weeks to complete.

    RAHC will reimburse the application fee upon your first placement with RAHC on the provision of your Working with Children Clearance Notice Number.

7. Insurance

  • Evidence of current professional indemnity insurance

Additional Documents Required

  • Evidence of name changes (if applicable)
  • Evidence of permanent residency if not Australian Citizen
  • Disciplinary Action: Provide details of any suspensions or actions from AHPRA and evidence of a certificate of Good Standing
